Applications

There are a few ways to fill out our application form, all includes signing up for an account with us. This allows us to relay messages and leaving contact information in our system. 

After signing up, the options are

1. Text us at 850-358-8535, let us know you have signed up! Leaving your name and what position you are applying for. Or what you like to collaborate with us. You will receive an email/sms link to fill out your application form. 

2. You may also do this exactly like above through emailing to admin@vaylawellness.com

3. You may go through the process in our booking link that will take you straight to the application. 

-Go into menu drop down for booking

-Click on the booking link 

-Choose other 

-Choose an application you would like to apply

-Follow the description all the way through 

-Choose any date and time

-Proceed to confirmation

-Use code BEWELL100

-Agree to terms & Book

-Submit your application 

-You have 15 minutes with this system

4. Come in our location and get a Physical Copy
